Centennial Pharmacy Services is an adherence-based pharmacy company that is partnering with organizations within the care continuum to provide medication management to the chronically ill and aging in place. Pulse 2.0 interviewed Centennial Pharmacy Services co-founder and President Lindsay Constantino to learn more about the company.
Lindsay Dymowski Constantino’s Background
What is Lindsay Constantino’s background? Constantino said:
“I come from a rich background in providing patient-focused pharmacy services, influenced heavily by my early experiences of growing up working alongside my parents in their pharmacy. This exposure not only provided me with deep industry insights but also shaped my passion for patient-centered care, which is at the core of everything we do at Centennial Pharmacy Services.”
Formation Of The Company
How did the idea for the company come together? Constantino shared:
“The idea for Centennial Pharmacy Services stemmed from recognizing a significant gap in the market for those who were on multiple medications and living with chronic conditions in the comfort of their own home, or choose to age in place rather than a facility. We realized that pharmacy make a significant positive impact on health outcomes and overall life. As the population ages, more people need pharmacy services that cater not just to immediate needs but to comprehensive, home-based care. We created Centennial to meet that need by providing specialized services that support patients in managing their medications effectively at home.”
“As the president, my primary responsibilities include strategic oversight, fostering key partnerships, and ensuring that our operations align with our mission to revolutionize patient care through innovative pharmacy solutions.”
Favorite Memory
What has been your favorite memory working for the company so far? Constantino reflected:
“Professional: One of my most poignant professional memories was receiving a letter from a woman after her father passed away. He had asked her to thank us for making his life so much easier after he started using our services. That letter touched me deeply, and I still keep it as a reminder of the profound impact we can have on people’s lives.”
“Personal: Working alongside my family—my mom, dad, and brother—has been incredibly fulfilling. Sharing an office with my dad and experiencing the unapologetic support and encouragement to excel every day from people who truly have my best interests at heart has been some of the most fun I’ve ever had. My all-time favorite memory at work was when I was pregnant with my second son. Feeling antsy at home, I went to the office with my yoga ball to sit at my computer and do some work. My dad played the happiest playlist and teased me the entire time, which made the time fly by. I gave birth the next morning and now I sing songs from that playlist to my son every night.”
Core Products
What are the company’s core products and features? Constantino explained:
“Our core offerings include comprehensive medication management, adherence packaging, medication synchronization and personalized clinical, pharmacy-based services, all tailored for patients who are managing their medications at home. We also emphasize a seamless integration with healthcare providers to ensure a holistic approach to patient care.”
Challenges Faced
What challenges have Constantino and the team face in building the company? Constantino acknowledged:
“Managing patient pharmacy care at home presents unique challenges, particularly in how we support patients and educate providers about our services, while still delivering top-tier care. One significant hurdle is that this model—long-term care pharmacy at home—is not yet formally recognized by the broader healthcare industry. This lack of formal recognition affects everything from how services are understood by other healthcare professionals to how they are reimbursed. To address these challenges, we’ve implemented comprehensive support systems for our patients. This includes not only managing medications but also providing direct coordination with healthcare providers to ensure integrated care.”
“We’ve also developed extensive educational programs for providers to help them understand the benefits and logistics of home-based pharmacy care, emphasizing its impact on patient outcomes. Beyond direct care and provider education, I am actively leading efforts to garner formal recognition and sustainable support for this type of pharmacy care. This involves spearheading educational initiatives, speaking at conferences, and participating on boards dedicated to this cause. These efforts are crucial for establishing long-term care at home as a recognized, reimbursable model of pharmacy care. The goal is to shift the industry’s perception and policies so that pharmacies like ours can be appropriately reimbursed for the comprehensive services we provide.”
“This model isn’t just about filling and billing; it’s about managing patient health as an integral part of the care team. It requires a different approach compared to traditional pharmacy services, offering a more patient-centered, outcome-oriented method that can significantly enhance the quality of life for those needing long-term care. Through these multifaceted efforts, we’re not only advocating for the recognition of long-term care at home but are also laying the groundwork for a sustainable practice that other pharmacies can adopt, ensuring that this innovative care model thrives and expands.”
Evolution Of The Company’s Technology
How has the company’s technology evolved since launching? Constantino noted:
“Since our inception, our prescription management software has seen substantial enhancements, incorporating features like ‘hard stops’ to improve the safety and accuracy of medication management. We’ve also advanced our delivery systems to provide real-time tracking and direct updates to patients, increasing transparency and trust. Our adherence technology has particularly evolved. Initially, we were limited to using a single pouch size and providing bilingual (English and Spanish) medication descriptions. Now, we’ve innovated to offer adaptable pouch sizes that can ‘grow’ or ‘shrink’ to fit an entire time’s medication into one pouch, significantly reducing confusion for patients managing multiple medications. Moreover, we’ve expanded the language options on these pouches to include dozens of languages, and we’ve added pictorial representations for patients who are illiterate. We also offer the flexibility to adjust the font size on these pouches, making it easier for patients with visual impairments to read their medication instructions clearly. Looking ahead, we’re developing new internal workflow technologies and planning to launch a comprehensive patient and provider portal. These steps are part of our ongoing commitment to using cutting-edge technology to enhance patient care and streamline healthcare provider interactions.”
Significant Milestones
What have been some of the company’s most significant milestones? Constantino cited:
“Our most significant milestones revolve around our groundbreaking partnerships with healthcare providers and our engagement in value-based care agreements. These collaborations have been transformative, allowing us to work closely with providers to enhance patient care significantly and improve their star ratings and health outcomes. Through these partnerships, we’ve been able to integrate our services deeply within the broader healthcare ecosystem, aligning our goals with those of providers to deliver patient-centered, outcome-focused care. Participating in value-based contracts has been particularly rewarding as it has enabled us to demonstrate our value in the healthcare world more concretely. These agreements have not only proven our ability to impact patient outcomes positively but have also shown our commitment to sustainable healthcare practices that prioritize patient well-being over volume. This shift towards value-based care represents a significant evolution in how pharmacy services are viewed and utilized in the healthcare industry, marking a major milestone in our journey to redefine the role of pharmacy care in patient management.”
Customer Success Stories
When asking Constantino about customer success stories, she highlighted:
“One particularly impactful story is that of an elderly patient who was on the verge of being placed in a nursing home due to frequent hospitalizations. After enrolling with us, our comprehensive medication management and personalized care coordination helped significantly reduce her hospital visits. This not only stabilized her health but allowed her to enjoy her independence at home, surrounded by family, which dramatically improved her quality of life.”
Funding/Revenue
When asking Constantino about the company’s funding and revenue details, she revealed:
“While specific financial metrics are confidential, I can share that Centennial Pharmacy Services has attracted significant interest from potential partners and investors due to our innovative long term care pharmacy at home model. However, we have been selective in our partnerships because it’s crucial that any collaboration honors our core mission of providing patient-centered care that is deeply integrated within the community. This model, while often more resource-intensive, is fundamental to producing the exceptional outcomes Centennial is known for. Our commitment to maintaining the integrity of our service model means that growth, though eagerly anticipated, must align with our values. We are excited about the future and the potential we have to expand with the right partners—those who understand and support our vision of community-focused, patient-first pharmacy care. We believe that with the right alignment, we will not only continue our trajectory of growth but also significantly enhance our ability to impact patient pharmacy care and health outcomes positively.”
Total Addressable Market
What total addressable market (TAM) size is the company pursuing? Constantino assessed:
Centennial Pharmacy Services is strategically targeting the significant and growing market of aging Americans, a demographic that is projected to increase markedly by 2030. This is often referred to as the ‘2030 problem,’ where the number of elderly individuals, particularly those over 65, is expected to rise substantially, leading to increased healthcare and living support needs. Our model is ideally suited to address this challenge by offering comprehensive, community-integrated, and patient-centered pharmacy care that allows individuals to remain in their homes while receiving the necessary medical support. Our services are designed not just to cater to this demographic’s medication needs but also to provide a holistic approach that includes medication management, adherence support, and coordination with healthcare providers. This model not only enhances the quality of life and health outcomes for our patients but also supports the broader healthcare system by reducing hospitalizations and healthcare costs. By focusing on this growing segment, we are looking at a TAM that will expand significantly in the coming years, potentially encompassing tens of millions of individuals. This represents a substantial market opportunity for Centennial Pharmacy Services, driven by demographic trends and the increasing preference among seniors to age in place. Our readiness to serve this market with innovative solutions places us in a strong position to capture and lead in this space.”
Differentiation From The Competition
What differentiates the company from its competition? Constantino affirmed:
“Centennial Pharmacy Services stands apart through our dedicated focus on integrating long-term care within the patient’s home, which is still a novel concept in pharmacy care. Our patient-first approach, which includes customized medication management and real-time coordination with healthcare providers, ensures that our services are not only proactive but also align with the highest standards of care. Furthermore, our commitment to compliance and regulatory leadership sets a benchmark in the industry.”
Future Company Goals
What are some of the company’s future company goals? Constantino emphasized:
“Our future goals are ambitious and centered around transformative growth. We aim to expand our services nationally, reaching patients in every state. We’re also focused on continually advancing our technology to enhance connectivity between patients, providers, and pharmacies. Additionally, we plan to cultivate new partnerships with health systems and insurers to integrate our model more deeply within the broader healthcare ecosystem.”

Any other topics you would like to discuss? Constantino concluded:
“One of my fervent hopes is that the broader healthcare industry will begin to recognize and appreciate the integral role of pharmacy, especially as it pertains to long-term care in the home setting. Pharmacies do much more than dispense medications; we are critical in managing patient health, preventing hospitalizations, and reducing healthcare costs through comprehensive medication management and personalized care. Additionally, I am passionate about discussing my role as a leader in long-term care pharmacy at home. This niche is not just a business sector—it’s a calling that combines innovation, patient care, and community service. Leading Centennial Pharmacy Services in this domain has given me unique insights into the challenges and opportunities of healthcare delivery outside traditional settings. I believe sharing these experiences can inspire further innovation and collaboration across the healthcare spectrum, driving us toward a more inclusive and effective system.”
