Conduit Health announced that it has raised $17 million in a Series A funding round to expand its vertically integrated platform that streamlines access to insurance-covered medical supplies for Medicare and Medicaid patients. The round was led by Drive Capital with participation from prior investors XYZ Ventures, Twelve Below, and Eniac Ventures, bringing the company’s total funding to $22 million.
The company is focused on simplifying a complex and often frustrating process faced by millions of patients who rely on insurance-covered home medical supplies. Despite an estimated 50 million Americans depending on these benefits, many encounter delays, denials, and administrative hurdles due to fragmented workflows involving physicians, insurers, and suppliers.
Conduit Health consolidates clinical evaluation, insurance authorization, and medical supply fulfillment into a single coordinated system. By aligning clinical workflows with reimbursement logic, the company aims to eliminate inefficiencies and reduce the time from prescription to delivery from weeks to a more streamlined process.
At the core of its platform is CareOS, an AI-powered authorization and reimbursement engine trained on more than 50,000 patient interactions. The system predicts coverage outcomes, automates payer workflows, and manages documentation across complex insurance environments. This enables Conduit to accelerate approvals, reduce administrative burden, and assume denial risk while scaling operations.
Since launching in late 2024, the company has delivered supplies to over 50,000 patients, expanded its product catalog fourfold, and built a payer network that includes more than 100 health plans covering nearly 90 million lives. The company also works with a range of referral partners, including home care agencies, skilled nursing facilities, and care coordination organizations, to further streamline access to essential equipment.
Looking ahead, Conduit Health plans to expand beyond durable medical equipment into additional insurance-covered services such as transportation to medical appointments, medically tailored meals, and home modifications, with the goal of improving access to a broader range of benefits for eligible patients.
KEY QUOTES:
“By unifying all of the clinical, administrative, and fulfillment work around an intelligent operating layer, we offer a care model that actually works for patients. Until now, the red tape in DME procurement—particularly in Medicare and Medicaid—has profoundly hindered access to these essentials. We built Conduit so the most vulnerable members of our communities can stay healthy and independent at home. The status quo simply doesn’t work for them, and as a company that isn’t something we are willing to accept.”
Natan Wise, Co-Founder And CEO, Conduit Health
“Conduit Health started with durable medical equipment because it’s historically been one of the most challenging and exhausting benefits for patients to access. Our mission is to make it easier for patients to access what they’re already entitled to, and our foundation was designed to ultimately expand into a broader range of insurance-covered services. We are building towards our vision of every eligible American having easy access to covered benefits beyond DME, like transportation to medical appointments, medically-tailored meals, and home modifications.”
Rocky Seftel, Co-Founder, Conduit Health
“The healthcare system makes it nearly impossible for individuals to obtain the home care equipment they need, covered by their insurance. Natan and Rocky identified this acute pain point, and the solution the team built feels like magic for the consumer. Conduit is easily engaging the hardest to reach patients in healthcare by simply making their lives easier, and it shows in how fast the company is growing.”
