Guideway Care: Waypoint Healthcare Solutions Acquisition Expands Patient Activation Intelligence Platform

Guideway Care announced it has acquired Waypoint Healthcare Solutions, integrating the company’s technology and team to expand its enterprise evidence-based patient activation intelligence platform and strengthen execution across the healthcare journey.

The transaction brings Waypoint’s team, including founders Lisa McDermott and David Bradshaw, into Guideway Care as the company accelerates development of a unified architecture designed to govern healthcare execution across pre-encounter and post-encounter interactions.

Guideway Care originally focused on providing journey management assistance to patients in disease-specific interactions. Over time, the company has expanded its approach into what it calls enterprise activation intelligence, combining behavioral science, workflow orchestration, technology, and human specialists to help ensure patients complete the recommended actions required for effective care.

The company said the acquisition strengthens its ability to connect healthcare strategies such as value-based care initiatives directly to measurable patient outcomes across areas, including access, referral closure, care transitions, chronic disease management, avoidable utilization reduction, and service line growth.

Healthcare systems and payers continue to face operational challenges even after significant investment in electronic health records, digital access tools, population health analytics, and enterprise artificial intelligence platforms. Guideway Care says many of these gaps occur between clinical recommendations and completed patient actions.

Across the industry, health systems continue to struggle with issues such as incomplete referrals, delayed follow-up care after hospital discharge, diagnostic findings that go unaddressed, lapsing preventive screenings, and digital demand that does not translate into scheduled care visits.

Guideway Care describes these breakdowns as activation failures occurring in the non-clinical layer of healthcare interactions, including before, between, and after patient encounters.

With the integration of Waypoint’s technology, Guideway Care said it will enhance its activation intelligence capabilities by combining human activation specialists with artificial intelligence agents that can identify risk signals, coordinate interventions, and manage patient pathways at scale.

The platform is designed to help healthcare providers and risk bearing organizations improve performance across metrics such as referral completion, readmission reduction, chronic condition management, emergency department utilization, preventive care gaps, and value based contract performance.
