Health Recovery Solutions Acquires Chronic Disease Management Company Rimidi

Health Recovery Solutions (HRS), a provider of remote patient monitoring and enterprise remote patient care platforms, announced it has acquired Rimidi, a digital health company focused on chronic disease management and remote monitoring, particularly in diabetes and cardiometabolic care.

The acquisition strengthens HRS’s efforts to expand its virtual care platform and provide more comprehensive chronic care management capabilities. By combining HRS’s remote patient monitoring infrastructure with Rimidi’s clinical decision support tools and expertise in ambulatory disease management, the companies aim to help healthcare providers deliver continuous, coordinated care across multiple conditions and care settings.

HRS has supported more than one million patients and works with hundreds of healthcare organizations across North America. The company launched its PatientFirst Pathways care model in 2025 to support connected, longitudinal care that follows patients from diagnosis through acute episodes and ongoing management at home.

Rimidi’s technology integrates clinical data from electronic health records with connected devices and patient surveys, enabling providers to better monitor patients with chronic diseases such as diabetes. Its platform also incorporates continuous glucose monitoring data from leading devices, including Dexcom, Libre, and Eversense, providing clinicians with actionable insights directly within EHR workflows.

The combination is designed to help healthcare organizations scale remote patient monitoring across multiple chronic conditions while simplifying clinical workflows and reducing the number of vendors required to manage patient care programs. HRS customers will gain access to Rimidi’s diabetes and cardiometabolic management capabilities, while Rimidi customers will benefit from HRS’s enterprise remote care infrastructure and expertise in post-acute care programs.

Rimidi’s platform has demonstrated measurable improvements in clinical outcomes and patient engagement. In one healthcare system deployment, the company reported an average 2.8% reduction in hemoglobin A1c levels among patients with type 2 diabetes along with reductions in hospitalizations among high-risk populations.

The integration of the two platforms will continue throughout the year, with both companies aiming to deliver a unified remote care solution that supports healthcare providers across the entire patient journey.
