Hopscotch Primary Care has raised $53 million in Series D financing to expand its technology-enabled primary care platform across rural communities after reaching more than 15,000 patients in the Southeastern U.S. Lead investors include 8VC and Town Hall Ventures. Existing investors aMoon Fund, Citi Impact Fund, Alumni Ventures and K2 HealthVentures also participated. New investors include Autism Impact Fund, Kleiner Perkins Chairman John Doerr, Heritage Provider Network founder Dr. Richard Merkin, and the Leon Levine Foundation.
Founded in 2021, Hopscotch currently operates 12 clinical locations, with its largest concentration in rural Western North Carolina.
The company plans to use the funding to deepen access within its existing region, enter additional rural markets and expand its technology-enabled operating infrastructure.
Hopscotch is addressing communities where physician retirements, hospital closures and other healthcare access challenges have made primary care increasingly difficult to obtain.
Its model includes same-day visits, 24/7 access to care teams and proactive patient outreach supported by technology designed to reduce administrative workloads for clinicians.
Hopscotch reported a Net Promoter Score of 89 and patient retention consistently above 90%.
The company also said its management of medical expenses has produced more than a 25-percentage-point improvement in medical loss ratios for patients over their first two years.
Hopscotch said those results, together with its operating model, have already resulted in profitable operations in Western North Carolina.
The company’s strategy combines in-person clinicians in rural communities with technology intended to increase their capacity and support value-based care.
The Series D will allow Hopscotch to bring that model to additional small towns while continuing to develop its clinical and technology infrastructure.
