The Collaborative for Healthy Rural America announced that Instacart has joined the organization as states begin implementing rural healthcare transformation initiatives supported by the Centers for Medicare & Medicaid Services.
According to the collaborative, Instacart’s addition expands CHRA’s ability to integrate food access and essential goods delivery into coordinated healthcare delivery models focused on chronic disease prevention and rural healthcare access.
CHRA currently includes collaborators such as Deloitte Consulting, Lumeris, Nuna, Teladoc Health, and Unite Us, with the group focused on helping states operationalize Rural Health Transformation initiatives funded through CMS cooperative agreements.
The organization stated that approximately $10 billion in first-year Rural Health Transformation funding has been awarded nationally, with states now moving from planning stages into implementation and performance measurement efforts.
Instacart said its participation will center around integrating nutrition access into healthcare workflows through Instacart Health initiatives designed to support chronic disease prevention and management in underserved rural communities.
CHRA stated that the collaborative’s broader operating model combines AI-enabled primary care, interoperable data systems, virtual care, predictive analytics, community referral networks, and social services coordination designed to help states modernize rural healthcare systems more efficiently.
The organization identified several recurring challenges highlighted across state rural healthcare transformation plans, including aging infrastructure, preventive care gaps, chronic disease burdens, provider shortages, and access limitations in rural communities.
According to CHRA, the collaborative’s technology stack includes AI-assisted outreach, virtual care systems, predictive analytics, and community referral platforms intended to support earlier interventions, improved care coordination, and expanded healthcare accessibility across rural populations.
The collaborative said states demonstrating measurable progress in access, utilization, and sustainability metrics by October 2026 will be positioned for continued federal funding support.
