Jukebox Health: Acquisition Of Braided Health To Expand AI-Driven Integrated Care Platform

Jukebox Health, a company focused on improving safety and independence at home for older adults and people with disabilities, announced it has acquired Braided Health, a care management technology company serving high-need, dual-eligible Medicare-Medicaid populations.

The acquisition is designed to expand Jukebox Health’s platform by integrating care management workflows with in-home clinical insights and interventions. By combining Braided Health’s AI-driven technology with Jukebox Health’s home-based care model, the company aims to help managed care organizations better manage costs and outcomes for complex patient populations.

Medicaid covers nearly 90 million Americans, with dual-eligible members accounting for a disproportionate share of healthcare spending. Health plans face increasing pressure to identify risks earlier and manage care more effectively, yet many rely on legacy systems that are not designed to address the complexity of these populations.

Braided Health’s CareAssist™ platform functions as an AI-powered “sidecar” that integrates with existing systems to surface actionable insights without requiring full system replacement. The technology supports care managers by streamlining caseload triage, identifying emerging risks, reducing documentation burden, and incorporating data from home-based and third-party sources.

Jukebox Health complements this capability by deploying clinicians directly into patients’ homes, where many cost drivers originate. Through in-home assessments, the company captures data on functional health and environmental conditions, translating these insights into targeted interventions. These may include enabling independent daily activities, deploying telehealth and remote patient monitoring, and addressing risks that contribute to avoidable hospitalizations and long-term care utilization.

The combined platform is intended to bridge the gap between identifying risk and acting on it, allowing earlier and more effective interventions. Jukebox Health reports that its approach has demonstrated repeatable return on investment by reducing costs tied to personal care utilization, hospitalizations, and institutional placements.

The acquisition also brings additional leadership to Jukebox Health, including Braided Health’s former CEO Jeff Grahling, who has experience across major health plans, and Emily Olson, RN, who adds clinical expertise in behavioral health and care operations.
