Kouper, a health technology platform designed to transform care transitions, emerged from stealth with $10 million in funding from General Catalyst, 25Madison, and CVS Health Ventures. The company’s goal is to support the millions of patients transitioning across healthcare settings annually in the US. This funding also included several prominent healthcare angel investors and will be used to grow further and develop the Kouper team and product.
