Nest Health: $22.5 Million Series A Raised To Advance Nationwide Expansion Of Whole-Family Medicaid Care

Nest Health has closed its Series A funding round at more than $22.5 million as the Louisiana-based provider of whole-family Medicaid care positions itself for its next stage of product development and multistate growth. The company, which delivers medical, behavioral, and social care directly into the home, has gained meaningful traction among families facing barriers to traditional care as well as among payers seeking improved outcomes and reduced costs.

Nest Health’s new investor lineup includes Socium Ventures, a venture and growth investment firm backed by Cox Enterprises, along with Amboy Street Ventures, Impact America Fund, Hopelab, University Growth Fund, and Luminary Impact Fund. Socium Ventures will also take a seat on the company’s board of directors. The fresh capital will support the expansion of Nest’s clinical offerings, increased use of artificial intelligence to streamline care team efficiency, and geographic growth across both current and future payer partnerships.

Founded to address the persistent gaps in Medicaid access for mothers, fathers, children, and postpartum women, Nest Health provides a comprehensive in-home model that integrates primary care, behavioral health, substance use treatment, social support, specialist coordination, virtual care, and 24/7 clinical access. The company leverages AI to automate administrative and clinical workflows, supporting care teams during and after patient interactions.

Nest Health’s outcomes in Louisiana and Arizona indicate significant performance improvements. Families in the program have demonstrated a two-to-one return on investment for payer partners. The company’s net promoter score is +98. Nest reaches three-quarters of families for whom health plans previously lacked accurate contact information. Participants have shown a sixty percent reduction in emergency room utilization after enrolling. Payer churn has dropped by more than half, to 55%. Vaccination rates in Louisiana have doubled relative to state targets, and in Arizona, sixty-two percent of Nest patients who received a flu shot this season had not received one the year prior.

The company’s expansion continues with new and existing partners that are turning to in-home care models to improve equity and stability for high-risk families.
