OffPlan Raises $2.5 Million To Expand Membership-Based Primary Care Platform

OffPlan announced that it has raised $2.5 million in seed funding to support the launch and expansion of its membership-based primary care platform.

The company is building a healthcare platform designed to help employers and employees access primary care through a model focused on affordability, physician accessibility, and reduced reliance on traditional insurance structures.

OffPlan plans to use the funding to support expansion across multiple states over the next two years. The capital will also help accelerate platform development, expand physician partnerships, support go-to-market operations, and launch services in additional states following initial market rollouts.

The company was founded to address what it sees as a structural problem in healthcare: the use of insurance for routine and predictable care. OffPlan’s model combines direct primary care memberships, transparent specialty pricing, and catastrophic coverage strategies to simplify healthcare access while lowering costs for employers and employees.

OffPlan said its platform is designed around physician-owned practices, same-day or next-day appointments, longer patient visits, virtual care access, chronic disease management, and transparent pricing without traditional primary care claims administration.

The company believes that a large share of healthcare utilization consists of routine and predictable care that can be managed more efficiently outside traditional insurance frameworks. By separating everyday healthcare from catastrophic coverage, OffPlan aims to reduce administrative complexity, lower costs, and improve the patient experience.

OffPlan is led by co-founder and CEO Greg Rable and co-founder and Chief Commercial Officer Omar Fernandez. Rable is a three-time founder and executive whose previous companies include Derivion, FactorTrust, and QuidMarket. Fernandez has more than 25 years of healthcare industry experience, including work in specialty pharmacy and physician practice support, and previously founded Sabio Health.

OffPlan is initially launching in Florida and Virginia, with additional market expansion planned nationwide. The company also plans to expand its physician network, employer partnerships, and advisor relationships as it scales.
