Pair Team, an AI-enabled medical group focused on the safety net, has acquired Town Square, a social care coordination company, as it pushes to expand technology-driven care models that integrate clinical, behavioral, and social care for Medicare and Medicaid beneficiaries.
The deal is intended to strengthen the infrastructure that connects health plans, providers, and community-based organizations, positioning the combined organization to coordinate complex care across settings with technology while keeping human teams focused on clinical judgment, hands-on care, and relationship-driven support.
Pair Team said the combined approach relies on an agentic work system and systems integration to enable an AI-driven care model in which technology actively coordinates care needs, while clinicians and community partners engage where they can serve their communities most effectively. The company also emphasized a continued focus on responsible AI use, outcomes measurement, and investment in community-based infrastructure.
As part of the acquisition, Town Square CEO and founder Royal Tuthill will join Pair Team as senior vice president of networks. In this role, he will lead the national expansion of Pair Team’s Medicare programs and deepen partnerships with community-based organizations and care hubs.

Town Square said its platform supports social drivers of health efforts at scale by enabling service coordination, data sharing, and reimbursement support across health plans, providers, community care hubs, and community-based organizations.
