Pear Suite, a digital health company focused on empowering community health workers (CHWs), announced it has raised $7.6 million in Series A funding to expand its AI-driven platform, scale its provider network, and extend its partnerships with Medicaid and Medicare health plans across the US. The funding round was co-led by Rock Health Capital and Nexxus Holdings, with participation from Mucker Capital, Enable Ventures, The SCAN Foundation, Acumen America, Impact Engine, and the California Health Care Foundation. The diverse investor participation underscores broad confidence in Pear Suite’s mission to redefine community-based care delivery.

The funding will accelerate Pear Suite’s efforts to equip thousands of community-based providers to serve the nation’s 78 million Medicaid members and an expanding Medicare population. The company’s platform enables health plans to scale culturally responsive and trust-based care models that improve outcomes, address social determinants of health, and reduce disparities in access to care.

Founded in 2021, Pear Suite helps CHWs deliver care that meets patients where they are—bridging medical, behavioral, and social support. The company’s technology platform provides workflow automation, reimbursement optimization, and real-time communication tools that allow CHWs and community-based organizations to collaborate seamlessly with health plans and healthcare systems. This integrated model enhances population health management and facilitates sustainable, value-based care at scale.

Pear Suite’s expansion comes amid significant shifts in U.S. healthcare policy, as Medicaid redeterminations, work requirements, and preventative care reimbursement reforms reshape the landscape of public health funding. It has also established partnerships with more than 25 health plans to implement community-based care strategies, align payers and providers, and advance health equity initiatives.
