Sage Care, a Palo Alto-based healthcare technology company developing AI-powered care navigation systems, has officially launched with $20 million in funding led by Yosemite and with participation from General Catalyst, Metrodora (Chelsea Clinton), OVTR.VC, SV Angel, Liquid 2, Seven Stars, Refract Ventures, AME Cloud Ventures, and Apollo Ono.

The company’s mission is to eliminate inefficiencies in healthcare navigation and patient coordination through clinically intelligent AI agents and advanced optimization algorithms designed to enhance operational performance and patient access.

Sage Care’s AI platform integrates directly into existing healthcare systems, leveraging predictive optimization and automation to streamline how patients are connected with care providers. Its solutions have demonstrated the ability to increase healthcare system revenue by 15–20% through more intelligent triage, specialist matching, and scheduling processes.

The system improves provider throughput by automating patient interactions and managing administrative burdens. Among its capabilities, Sage Care’s AI agents can:

  1. Answer and triage patient calls, messages, and requests around the clock.
  2. Manage appointment scheduling, referral processing, and insurance verification.
  3. Coordinate follow-ups for diagnostics, care plans, and medication adherence.
