Yosemite led the round with support from General Catalyst, Metrodora (backed by Chelsea Clinton), OVTR.VC, SV Angel, Liquid 2, Seven Stars, Refract Ventures, AME Cloud Ventures, and Apollo Ono.
Based in Palo Alto and founded in 2024, Sage Care deploys clinically intelligent AI agents and advanced optimization algorithms to streamline patient interactions with the healthcare system. Its platform has already shown potential to help health systems increase revenue by 15–20 % through improved care coordination, accurate specialist matching, optimized scheduling, and triage.
The solution tackles administrative bottlenecks by enabling providers to: handle patient calls and messages around the clock, manage appointment scheduling, referrals, and insurance verification, and coordinate follow-up for diagnostics, care plans, and medication adherence. Agents are customizable, rapidly deployable within 48 hours, and compatible with existing hospital and clinic workflows and policies.
Sage Care already serves regional multispecialty clinics and large health system partners, including Jiva Health, with upcoming deployments planned at Bronson Healthcare and White Plains Hospital. The company emphasizes that by streamlining the front office of healthcare, providers increase appointment volume and reduce revenue loss from no-shows and mismatches.
