Tarsus Pharmaceuticals announced it has acquired clinical-stage ophthalmic biotechnology company iRenix Medical, adding late-stage investigational therapy IRX-101 to its eye care pipeline. The deal strengthens Tarsus’ presence in retinal disease while expanding beyond its commercial product XDEMVY into therapies designed to improve the patient experience during vision-preserving procedures.
IRX-101 is an investigational ocular antiseptic developed to reduce post-procedural pain and corneal toxicity in patients undergoing intravitreal injections, one of the most common treatments for chronic retinal diseases. According to Tarsus, more than 11 million intravitreal injections are performed annually in the U.S., yet physicians have relied on povidone-iodine as the standard antiseptic for more than four decades despite its association with eye irritation and corneal damage.
The acquisition provides Tarsus with a late-stage clinical asset supported by positive Phase 2b/3 data and an FDA-aligned development plan. The company expects to begin a Phase 3 study during the first half of 2027, with results anticipated in 2028.
In the completed Phase 2b/3 RELIEF trial involving 154 patients, IRX-101 achieved statistically significant improvements over povidone-iodine on both co-primary endpoints. Patients receiving IRX-101 experienced approximately a 50% relative reduction in post-procedural pain, with half reporting no pain following treatment. The study also demonstrated roughly a 25% relative reduction in corneal fluorescein staining, indicating less damage to the corneal surface.
Tarsus believes the therapy could address an unmet need for patients with chronic retinal diseases such as neovascular age-related macular degeneration, diabetic macular edema, retinal vein occlusion-related macular edema, and geographic atrophy, all of which often require injections every four weeks to preserve vision. By reducing discomfort associated with repeated procedures, the company believes IRX-101 could help improve treatment adherence while establishing a foundation for its expansion into retina.
Under the terms of the agreement, Tarsus will pay approximately $75 million upfront, split evenly between $37.5 million in cash and $37.5 million in Tarsus common stock. The agreement also includes up to $490 million in potential regulatory approval and commercial milestone payments tied to the future success of IRX-101.
The transaction reflects Tarsus’ broader strategy of building a diversified eye care company focused on therapies that address underserved areas of ophthalmology. Alongside XDEMVY, the company is advancing TP-04 for ocular rosacea and TP-05 for Lyme disease prevention, both currently in Phase 2 development.
