UT Medical and TransformativeMed have formed a development partnership to create an agentic AI-powered discharge planning solution intended to streamline one of the more complex workflows in acute care.
The jointly developed system will become part of TransformativeMed’s CORES Platform, which the company describes as an Agentic Clinical Operating System.
The collaboration combines UT Medical’s clinical operations expertise with TransformativeMed’s agentic AI technology to reduce administrative burden on care teams and improve coordination during patient discharge.
UT Medical and TransformativeMed plan to showcase the solution at the Oracle Health Summit from September 22 through September 24, 2026.
TransformativeMed’s CORES platform integrates intelligent coordination and automation directly into Oracle Health EHR workflows, helping health systems coordinate clinical information and automate workflow tasks.
UT Medical is East Tennessee’s academic health system and operates a 710-bed nonprofit hospital in Knoxville along with a broader network of primary and specialty care practices, outpatient centers and urgent care locations.
