Zócalo Health: $15 Million Series A Raised To Scale Community-Based Primary Care Model

Zócalo Health, a tech-enabled, community-based primary care provider, announced a $15 million Series A funding round, bringing its total funding to $22.75 million. The oversubscribed round was led by EO Ventures, with participation from Talipot and existing investors including Vamos Ventures, Animo Ventures, Acumen America, Sorenson Ventures, BarronKent, and Kapor Center.

The company plans to use the new capital to expand nationwide, grow its community-based workforce, and deepen partnerships with health plans. Zócalo Health partners with organizations such as Anthem Blue Cross and Health Net to deliver care to high-need patient populations.

Zócalo Health focuses on patients with complex medical, behavioral, and social needs who account for a disproportionate share of healthcare costs and are often difficult to engage. Its model combines community-based care teams with virtual clinical services to improve engagement, outcomes, and cost efficiency within value-based care frameworks.

The company has demonstrated strong traction, including approximately 4x year-over-year revenue growth, expansion from two regional health plan partnerships in 2024 to more than a dozen in 2025, and entry into multiple new markets. It also reports high member retention, frequent engagement, and patient satisfaction scores exceeding industry benchmarks.

Zócalo Health’s care model is centered around community health workers, or promotoras de salud, who are embedded within the communities they serve. These teams work alongside primary care providers, behavioral health clinicians, and care coordinators to deliver continuous, relationship-based care. Behavioral health services are integrated into the model, addressing rising patient needs for mental health support alongside medical and social care.

The company also leverages a proprietary technology platform, including AI-driven tools, to support frontline teams, enhance care delivery, and help patients navigate coverage and eligibility requirements such as Medicaid redetermination.

Zócalo Health’s partnerships with Medicaid managed care organizations align with broader state initiatives like CalAIM in California, positioning the company to scale its whole-person care model across additional states over time.
